Fewer realize it also affects how we speak. When the body senses threat, real or perceived, it shifts into a state of heightened alertness. Breathing becomes shallower, muscles tense, and the whole system prepares to react quickly. The voice and speech mechanism sit right in the middle of that response, which is why so many people notice changes in how they communicate during periods of stress or anxiety.

How stress shows up in speech and voice

The link isn’t just anecdotal. Ask any speech-language pathologist and they’ll tell you they see it constantly in session:

Increased disfluency, including more blocks, repetitions, or prolonged sounds, especially for people who already stutter;

A voice that becomes strained, hoarse, or breathy, sometimes even disappearing altogether in moments of acute stress;

Difficulty finding the right word mid-sentence, even for common, everyday vocabulary;

Trouble following fast or complex verbal information, particularly in noisy or high-pressure settings;

A faster, less controlled speech rate that can be hard to slow down consciously.

Why does this happen?

Each of these has a physiological explanation behind it:

Stuttering and disfluency: anxiety increases muscle tension, which raises the frequency of blocks, repetitions, and prolonged sounds;

Voice strain: involuntary tightening of the laryngeal muscles under stress can make the voice hoarse, tense, or muffled, and in some cases, cause a sudden loss of voice;

Word-finding trouble: rising cortisol affects the prefrontal cortex, the part of the brain responsible for working memory and word retrieval, which is why the right word can suddenly feel out of reach;

Trouble processing speech: stress puts the nervous system on high alert, and that hypervigilance can work against the brain’s ability to quickly take in and process verbal information.

What does speech-language pathology offer?

These patterns are not fixed, and that’s the part worth holding onto. Because speech-language pathologists work directly with the physical and strategic sides of communication, they’re often the ones best placed to help when stress and anxiety start showing up in someone’s voice or speech.

A few of the tools they turn to:

Diaphragmatic breathing, to shift away from the shallow chest breathing that stress tends to trigger and support a steadier vocal flow;

Gentle vocal onset techniques, which reduce the force with which the vocal folds come together and ease laryngeal tension;

Pacing and pausing strategies, which turn silence into a tool for the speaker instead of something to avoid or fill anxiously;

Gradual, supported exposure to challenging speaking situations, such as phone calls or public speaking, so confidence builds step by step.

The evidence behind it

Breathing work in particular is backed by more than clinical experience. A controlled study on healthy adults found that a structured breathing intervention led to lower cortisol levels and reduced negative affect compared to a control group. It’s not just a relaxation habit, there’s a measurable effect behind it.

Speech-language pathologists focus on how stress and anxiety show up in communication, whether that’s a tighter voice, more disfluency, or trouble finding words. In some cases, that’s where the work stops and it’s enough. In others, addressing what’s driving the stress in the first place makes just as much of a difference, which is why a complementary approach through psychology, cognitive behavioural therapy, or techniques like heart rate coherence is sometimes recommended alongside speech and voice work. The two aren’t in competition, they simply address different parts of the same picture.

Normal nerves, or something worth a closer look?

A racing heart before a big presentation, tripping over your words during a job interview, going blank for a second when a manager asks a tough question on the spot… These moments are part of being human, and they happen to nearly everyone at some point.

What’s worth paying closer attention to is when this becomes more than an occasional slip: speech difficulties, voice changes, or word-finding struggles that show up regularly, get worse over time, or start to hold you back at work or in everyday conversations. At that point, it’s less about nerves and more about a pattern worth talking through.

If any of this sounds familiar, you are not imagining it, and you are not alone. Our speech-language pathologists at Centre Mosaïque in Quebec City can help you understand what’s happening physically when stress takes over your speech, and work with you on concrete strategies to feel more grounded the next time it matters most.

Sources: Iverach, L., & Rapee, R.
